The Federal Occupational Safety and Health Administration received a total of 20,853 US dollars to Power Flame 2001 p. 21st for six violations that were determined in an investigation after the death of an employee at the beginning of this year.
The 24 -year -old Shawn Sexton died on January 24 at Labette Health after he had no longer reacted to Power Flame on the powder coat floor.
Osha monitors security and health problems at the workplace and opened an investigation after Power Flame had reported the death of Sexton.
The OSHA initially evaluated $ 29,790 in terms of punishments in the case, but this changed to 20,853 US dollars.
An OSHA report shows six quotes, four of which are serious.
1: Ernst; $ 4,468.80 punishment. The Osha summary states that Power Flame did not rate the breathing flags in the workplace. On January 24, the employer did not lead any personal air sample for employees in the powder coating areas where employees wear respiratory protection masks.
2: Ernst; $ 5,957.70 punishment. Compressors that deliver breathing coats were not built and located so that contaminated air could not get in. The intake of the Sullair Industrial Air compressor was in the parking lot of the employees, on which toxic exhaust gas vapors could be drawn into the system, the Osha report says.
3: Ernst; USD 10,426.50. The OSHA report says that a three-way valve that combines nitrogen and compressed air systems enabled the possible introduction of nitrogen gas into the breathing air lines in the powder shift cabin. The autopsy report showed that Sexton accidentally died of causes, including the inert gas embroidery by inhaling nitrogen gas.
4: Ernst; No punishment. The report states that the breathing air on January 24 did not contain 19.5% to 23.5% oxygen. The compressed air did not meet the requirements for breathing when the oxygen content fell below 19.5%.
5: others; No punishment. Repairs on an air hood delivered by Allegro followed the recommendations and specifications of the manufacturer due to a change in the hose of the waist belt assembly with a hose connection section that is not approved by Allegro, according to the OSHA report.
6: others; No punishment. This refers not to display a day in connection with the maintenance of the compressor.
In an e -mail to Sun, General Manager from Power Flame offered an answer to an inquiry from the sun.
Brian Meek wrote that Power Flame was “deeply sad” by team member Sexton's death.
