The death of a woman at the HEB warehouse leads to federal and local investigations

SAN ANTONIO – Federal authorities are investigating why a woman who worked at an HEB warehouse died on the job last month. We have confirmed that the Occupational Safety and Health Administration (OSHA) has opened an investigation into the death of 27-year-old Teresa Dominguez.

Dominguez died Oct. 24 at the warehouse on the 5700 block of Rittiman Road on the Northeast Side. She was found unconscious in a freezer and later pronounced dead at the hospital.

According to initial police reports, she died in a forklift accident. HEB disputed that report and said her death was due to what it called a “medical incident.”

The Bexar County Coroner's Office confirmed that Dominguez died of “blunt trauma to the lower extremity.”

HE-B's statement differs from the police report, which indicates that Dominguez was found unconscious in a freezer and may have been injured by a forklift.

“Our HEB family is heartbroken and we are supporting and keeping in touch with the family during this difficult time. We also remain close to our partners,” HEB said in a statement.
